Just because a surgeon's practice revolves around putting implants in does not mean they are skilled at explanting.
What is En Bloc? En Bloc is an Oncological term, however it is sometimes confused with capsulectomy. As published in the Aesthetic Surgery Journal Open Forum, Volume 1, Issue 1, March 2019, the "standard of care treatment for the majority of patients with breast implant-associated anaplastic large-cell lymphoma (BIA-ALCL) is surgical resection...... An en bloc resection involves a total capsulectomy, explantation, complete excision of associated masses, and excision of any involved lymph node(s)."
We recommend that you find a surgeon for your explant who will do a complete capsulectomy, ideally with the complete capsule removed intact, with the implant still inside. Once the capsule is removed from the chest cavity, it is then opened to extract the implant.
Many Plastic Surgeons will not guarantee complete capsulectomy, but will indicate that it is their objective. The procedure is more difficult when the capsule is thin and is adhered to the ribcage, in which case the capsule will need to be lasered in order to remove it.
The important thing is that you make an informed decision about which Surgeon you will use.
